I am accepting limited new patients. Unless I have already accepted you as a

client questionnaire available at www.vakkur.com,
which you can fax (877-512-4854) or email (mvakkur@gmail.com)
to me for review. 1. Choose your top 3 choices from the
30-minute time slots below labeled "OPEN" and email me with those
choices (please put "SCHEDULE" in the subject line). Note that first time visits (note: I am accepting limited new patients) require
2 consecutive half-hour slots. 2. Please include in the body of the email
following your time slot requests your responses to the follow-up
questionnaire which should be completed before each visit. |

Do not make an appointment unless you are prepared to give me at least 24 hours notice if you subsequently need to cancel. 6. If you must cancel an appointment on the

soon as possible. I may waive or reduce
the fee for a missed appointment if a good-faith attempt is made to notify me
immediately. |
