following your time slot requests your responses to the follow-up questionnaire which should be completed
before each visit. 2. If you have not already done so,
please review and acknowledge your consent of the new
Practice Policy as of June 1, 2009.

404 964 9883. 4. Once I have emailed you back with confirmation of an appointment, please pay for this appointment via the web site (choose the appropriate time slot – the vast majority should be and pay by credit or debit card. |

5. Do not make an appointment unless you are prepared to give me at least 24 hours notice if you subsequently need to cancel. 6.
If you must cancel an appointment on the same day because of an
emergency or illness, please EMAIL or FAX YOUR CANCELATION as soon as
possible. I may waive or reduce the
fee for a missed appointment if a good-faith attempt is made to notify me
immediately. |
